Personal Health Assessment

Step 1: Online health survey

Grab your smartphone or computer and log in to your secure wellness portal; or create your account using your full name and date of birth. Once inside, the survey will ask about your lifestyle, health habits, and overall wellbeing. Takes about 10 minutes.

Step 2: On-site biometric screening

A registered nurse will measure your blood pressure, heart rate, abdominal girth, and BMI. They’ll also take a small sample of blood through a finger stick (no blood draw!) to get your cholesterol, triglyceride, and blood glucose numbers within minutes. Don't worry—you don’t need to fast beforehand. Your nurse will explain your results, answer your questions, and offer useful recommendations and resources to help you live your best life. Takes 20 minutes.

1:1 Wellness Coaching

Set and reach your goals with the help of a registered nurse, nutritionist, exercise specialist and/or stress and mindfulness expert. Build a fitness routine that works for you, pick up new nutrition tips, and get better equipped to cope with everyday pressures. The following coaching programs are available by phone or video call.

Condition Management

If you’re living with a chronic health condition, condition management coaching will provide you with the specialization and expertise you need to create a personalized wellness plan. Coaching is offered for the following:

Are you ready to commit to making real change in your life? Develop the mindset, motivation and skills needed to lose weight now and keep it off forever. This program will help you make simple, healthy changes in how you think, how you feel and what you do in relation to food. December – Put Down Stress

Stress affects your mental, emotional and physical wellbeing. Identify and understand your stressors and learn techniques for handling stressful events. This session will offer self-management strategies to literally "put down" the stress, resulting in more balance, stability and personal satisfaction.

Programs are open to individuals who participate in a Partners + Napier Health Care plan.
All Partners + Napier employees are eligible to complete a Personal Health Assessment, including a biometric screening. In addition, employees enrolled in a Partners + Napier sponsored health insurance plan are eligible to join a condition management program.
Spouses or domestic partners enrolled in a Partners + Napier sponsored health insurance plan are also eligible to complete a Personal Health Assessment, including a biometric screening and join a condition management program.

Programs are open to individuals who participate in an Optimax Health Care plan.
All Optimax employees are eligible to complete a Personal Health Assessment, including a biometric screening. In addition, employees enrolled in an Optimax sponsored health insurance plan are eligible to join a condition management program.
Spouses or domestic partners enrolled in an Optimax sponsored health insurance plan are also eligible to complete a Personal Health Assessment, including a biometric screening and join a condition management program.

Eligibility & Incentives

Regular full-time and part-time associates and spouses or domestic partners enrolled in a Thompson Health medical insurance plan are eligible to participate in UR Medicine Employee Wellness programs and receive incentives.

Eligible associates/spouses or domestic partners can receive up to $450/$375 per calendar year:

  • $150/$125 for completing the Personal Health Assessment, including the online health survey & biometric screening.
  • $150/$125 for each personal wellness coaching program (lifestyle management and/or condition management) and/or group program(s) completed (up to a maximum of two program completion incentives).

Weight Loss for Life counts as two program completions. If another incentive eligible program is completed and paid out before Weight Loss for Life program completion and incentive payout, associates/spouses or domestic partners will earn $150/$125 upon Weight Loss for Life program completion.

Incentives are paid quarterly after program completion: March 28, June 20, September 26, December 19. Programs must be completed by November 30, 2024 to receive your incentive in 2024.

Incentives are paid as cash, HSA or 403(b) contribution. Select your payout preference upon health survey completion.
Must be an enrolled associate/ spouse/domestic partner on our health plan to receive quarterly incentive payouts.

Eligibility & Incentives

All Full Time and Part Time Permanent Livingston County employees are eligible to participate in UR Medicine Employee Wellness programs. Spouses enrolled in the County health care plan are eligible to participate in the Personal Health Assessment and condition management coaching.

Individuals who complete the full Personal Health Assessment, including the online health survey and on-site biometric screening, by May 31, 2024 will receive a $100 gift card.
